
To obtain a passport for the first time, you will need to go in person to one of the 9,000 passport acceptance facilities located throughout the United States. Acceptance facilities include many federal, state and probate courts, post offices, some public libraries and a number of county and municipal offices. THE CITY OF DAVISON IS A PASSPORT ACCEPTANCE FACILITY. There are also 13 regional passport agencies, which service customers who are traveling within two weeks (14 days) or who need foreign visas for travel. Appointments are required in such cases.
Once again, you must apply in person if you are:
- applying for a U.S. passport for the first time
- if your expired passport is not in your possession
- if your previous U.S. passport has expired and was issued more than 15 years ago
- if your previous U.S. passport was issued when you were under 16 years of age
- if your currently valid U.S. passport has been lost or stolen
Passport applications are available at City Hall between the hours of 9:00 a.m. and 4:00 p.m.
As of February 1, 2008, you can now start applying for the new Passport Cards (please visit www.travel.state.gov for more information). These cards are wallet sized cards that can be used for land and sea travel ONLY between the U.S and Canada, Mexico, Bermuda and the Caribbean ONLY. The requirements for obtaining the card are still the same as the passport book. The fees are explained below. This card can be very useful if you are a frequent traveler to Canada, due to its size, cost and convenience. Although you can apply for the cards now, they will not be printed or mailed until Spring 2008.
Things you need to bring with you:
- passport pictures
- valid driver’s license or State issued I.D.
- original birth certificate
- check book, money orders or cash (for City portion only)
- expired passport (if applicable)
Requirements for Children under 16 years of age:
Each minor child must appear in person. Both parents and/or legal guardians signatures are required on a passport application for children under 16 years of age. If one parent cannot appear to sign the application, a
Statement of Consent DS-3053 is to be notarized and filled out.

Requirements for Children between the ages of 16 and 17 years old:
Each minor child must appear in person. If your minor child does not have identification of his/her own, the parent must accompany the child and present identification.
Cost Requirements:
Passport Book:
- Age 16 and Over: $100.00 Separate checks ARE REQUIRED for the Department of State ($75.00, by check, money order or cash) and the City of Davison ($25.00, by check, money order or cash ).
- Under 16 years of age: $85.00 Separate checks ARE REQUIRED for the Department of State ($60.00, by check, money order or cash) and the City of Davison ($25.00, by check, money order or cash).
Passport Card:
- Age 16 and Over: $45.00 Separate checks ARE REQUIRED for the Department of State ($20.00, by check, money order or cash) and the City of Davison ($25.00, by check, money order or cash ).
- Under 16 years of age: $35.00 Separate checks ARE REQUIRED for the Department of State ($10.00, by check, money order or cash) and the City of Davison ($25.00, by check, money order or cash).
Applying for the Passport Book and Card at the same time:
Cost for first time applicant:
- Age 16 and Over: $120.00 Separate checks ARE REQUIRED for the Department of State ($95.00, by check, money order or cash) and the City of Davison ($25.00, by check, money order or cash ).
- Under 16 years of age: $95.00 Separate checks ARE REQUIRED for the Department of State ($70.00, by check, money order or cash) and the City of Davison ($25.00, by check, money order or cash).
DELIVERY OF PASSPORTS IS GENERALLY FOUR (4) to SIX (6) WEEKS
If you care to expedite your passport, there is an expedite fee. The additional fee is $60.00 per passport and an additional fee for two-way Overnight Delivery. Expedite fees DO NOT include overnight delivery. You, as the customer, must arrange for overnight delivery to ensure delivery within three (3) weeks.
**If you are leaving in four (4) weeks, it is recommended that you expedite your application to ensure that you receive your passport in time.**
Traveling to the Caribbean, Bermuda, Panama, CANADA or MEXICO?
January 23, 2007 A passport is a REQUIREMENT for all air travel to or from the Caribbean, Bermuda, Central American, South America, CANADA and MEXICO.
Summer 2008 A passport is a REQUIREMENT for all air, sea and land border crossings.
These changes have been rolled out in phases to provide as much advanced notice as possible.
